Acknowledgement of Risks, Injury & Obligations

You acknowledge that the activity you are to undertake is potentially dangerous and that by participating in it, you are exposed to certain risks. You acknowledge and understand that whilst participating in such activity; You may be injured physically or mentally, or die; Your personal property may be lost or damaged; Other persons participating in such activity may cause you injury or damage your property; You may cause injury to other persons or damage their property; The conditions in which the activity is conducted may vary without warning; You assume the risk of and responsibility for any injury, death, or property damage resulting from your participation in the activity.

Release and Indemnity

You acknowledge the risk associated with participating in fitness activities and exercising. Your participation in any activity with Forge Performance is completely voluntary, and you acknowledge that you are assuming all risks of injury to yourself or others, including illness or medical condition. Please raise any concerns about starting an exercise or fitness program with your physician before beginning any activities with Forge Rehab and Performance. You agree on your behalf (and on behalf of your representative, heirs, estate trustees or assigns) to a) release, indemnify and discharge Forge Rehab and Performance. Including its owners, officers, directors, agents, employees, or independent contractors (“us”), from all claims or causes of actions (known or unknown) which you may have arising out of our negligence, including negligence of our staff, agents, or representatives, and b) to indemnify and save us harmless from any claims or causes of action (known or unknown) brought against us by any party arising out of your actions, including your negligence, while participating in fitness or exercise activities with Forge Rehab and Performance or elsewhere.

No Warranty

We make no warranty or guarantee regarding the facilities and services available to you under this agreement.

No Verbal Agreements

There are no promises, representations, understandings or agreements between us other than as set out in this agreement. Any changes must be made in writing and signed by both you and Forge Rehab and Performance


Social Media

You agree to have your son/daughter feature in photos/videos of the training sessions with Forge Rehab and Performance. If you wish for your son/daughter to not appear in any such videos/photo please contact our athlete experience team [email protected] Used for social media and website information.
